For new participants interested in joining, we have a two step process before acceptance to our program:
Step 1: Complete the Assessment Questionnaire
Step 2: Return completed assessment questionnaire to our program coordinator, at info@facealberta.ca who will then be in contact with you to arrange a one-on-one consultation/assessment to discuss programming options based on your goals.
For current participants (those who have already had an assessment with one of our conductors):
For those aged 18+, fill out our Registration Form for Adult Program and our Adult Assessment Questionnaire
For those under the age of 18, fill out our Kids Registration Form 2018-19
