For new participants interested in joining, this is the process:

Step 1: Complete the appropriate Assessment Questionnaire:

Adult Day Program

Step 2: Send the completed questionnaire to info@facealberta.ca.

Step 3: We will review, and be in touch to set up an assessment with our Conductors.

We direct bill FSCD for BDS contracts and Specialized Respite hours.
